Thank you for your willingness to allow your child onto a field trip with Sacred Heart Academy! You should usually receive the permission slip form from your child’s teacher or the leader of the field trip. However, if a need arises, below is a blank field trip permission form. This form can be used by you to complete a field trip permission form. Please carefully follow instructions from your child’s teacher or the leader of the field trip:
– All of the information you complete on the form should be very specific and accurate to what the leader of the field trip requested.
– If you have multiple trips, with multiple children, on the same day, the Academy needs a separate form for each child because the leader of the field trip needs to carry the form with him or her during the field trip for emergency purposes.
